The office personnel will distribute the student and family re-enrollment form and emergency medical form for all families approximately one month prior to the re-enrollment early discount deadline which is March 31 for the following year.

• Student Driver Form (PDF) - necessary for all student drivers
• Automatic Debit Form (PDF)
• Food Allergy Questionnaire Form (PDF) - only necessary if your child has an allergy
• Food Allergy Care Plan (PDF) - only necessary if your child has an allergy (both Food Allergy Questionnaire and Food Allergy Care Plan both needed)
• Field Trip/Athletic Transportation Release Form - must be turned in before your child goes on a field trip or goes to an athletic event in which he/she is on the team.
